One thing that I believe proves this is tool-assisted is at 5:21, he shoots two fireballs at almost exactly the same time, followed by another two at the same time, then again to finish off the mini-boss. He does it again at 6:33.
It's a speed run... sort of faked... these guys run the games a frame at a time in a virtual box to get the best possible times... go check it out, some amazing stuff!
