Roth UI World of Warcraft

Hey, i just downloaded ur UI and i think its amazing but i got a few problem i would like to ask.
Personally i rolled as a druid for my main, the bar that indicates all my forms were no longer there when i activate ur UI, are there any solution to these? Secondly, i cant seem to see all the damages i done, are there any solution to these? i hope u can help me with this problem.. Thanks
