One thing I noticed about Outland is that there's a marked reduction in ninja'ing even in PUGs. And of course, Heroics are even better - folks who aren't Heroics ready die very quickly and don't even get a chance to ninja, so generally you're playing with knowledgable players.
Of course, crafting one's own gear (if you're a Mage or Priest or Warlock) can also reduce the distress if there IS a ninja... but thankfully I'm glad to say I haven't run into many as of late. :)
